Provide resident out-of-hours medical cover across Adult, Old Age, CAMHS, Learning Disabilities, Forensic, PICU, and Mother & Baby services. Perform full admission clerking for new inpatient admissions, including psychiatric history, Mental State Examination (MSE), physical examination, capacity assessment, risk assessment, VTE assessment, ECG interpretation, and documentation on PARIS EPR. Conduct Section 136 (Mental Health Act) triage assessments in A&E and Place of Safety suites; liaise with AMHPs, registrars, and consultants to support safe decision-making and onward planning. Manage acute physical health problems in psychiatric inpatients (e.g., falls, infections, reduced consciousness, chest pain), escalating appropriately to acute hospitals and emergency services when required. Prescribe safely using EPMA, including psychotropics, PRN rapid tranquilisation, diabetic medications, nicotine replacement therapy, vitamin D supplementation, and emergency medications. Respond to medical emergencies and crash calls within inpatient psychiatric settings and support urgent transfer to acute care when indicated. Complete discharge documentation and transfer letters to ensure safe continuity of care with GPs, community teams, and acute services. Participate in structured morning and evening on-call handovers via Microsoft Teams; prioritise and communicate outstanding tasks clearly. Work collaboratively within MDTs, including nursing staff, crisis teams (CATT/CRHTT), safeguarding teams, and social services.

Shadowed consultants and MDTs in adult inpatient psychiatry and contributed to ward round discussions. Took supervised psychiatric histories and conducted Mental State Examinations (MSE). Observed risk assessment, capacity assessment, and psychotropic medication management. Attended multidisciplinary meetings and handovers to support care planning.

Assessed patients with dementia (including early-onset), depression, anxiety, delirium, and behavioural and psychological symptoms of dementia (BPSD). Performed cognitive assessments including MMSE, MoCA, ACE-III, and GDS under supervision. Participated in ward rounds, MDT and CPA meetings with nurses, OTs, social workers, and relatives. Contributed to risk and capacity assessments under supervision and gained exposure to safeguarding concerns (e.g., financial/elderly abuse and neglect). Became familiar with PARIS EPR and Civica e-drug chart systems; monitored for side effects of psychotropic medications.

Observed assessment and management of acute medical and surgical presentations in A&E. Took supervised histories and examinations and supported team workflow where appropriate. Gained proficiency in NHS systems including iPortal, Careflow and PACS. Attended MDT discussions and handovers; developed cultural competence working with diverse patient populations.

Managed common vascular conditions under consultant supervision including peripheral artery disease, varicose veins, aneurysms, deep vein thrombosis, chronic venous insufficiency, critical limb ischaemia, dialysis access issues, wound care and vascular trauma. Conducted OPD consultations and inpatient reviews; formulated and implemented treatment plans. Prepared patients for surgery and provided post-operative care, including monitoring for complications and pain management. Assisted in procedures such as arteriovenous fistula creation, carotid body tumour excision, sclerotherapy, Trendelenburg procedure, and endarterectomies. Led an audit on superficial vein preservation rates in CKD patients awaiting AV fistula creation and produced patient education materials.

General Surgery & Allied Specialties (6 months): Conducted supervised ward rounds and documented clinical findings and procedural notes. Managed admissions and discharges; supported care in critical care and general wards. Provided initial assessment and management for surgical emergencies including acute abdomen, bowel obstruction and trauma-related injuries. Assisted with pre- and post-operative patient care and theatre exposure. Performed clinical eye examinations (visual acuity, visual fields, fundoscopy) and supported labour room duties including CTG monitoring and postpartum care. General Medicine & Allied Specialties (6 months): Conducted comprehensive histories, physical examinations and Mental State Examinations for a broad range of medical presentations. Monitored vital signs, recorded patient progress, and maintained accurate medical records. Assisted in diagnostic and interventional procedures including ECGs, echocardiograms, stress tests and angiography support. Ordered and interpreted laboratory and imaging investigations under supervision. Administered medications and monitored therapeutic response and adverse effects. Performed venepuncture, IV cannulation and suturing; triaged and stabilised patients in emergency settings prior to referral. Delivered departmental presentations including 'Malabsorptive Diarrhoea' and 'Bacterial Skin Infection'. Supported rota coordination and maintained monthly duty rosters for interns.
