As a Support Worker at Thames Reach, I contribute to the mission of helping vulnerable individuals find homes and lead fulfilling lives. In this role, I'm undergoing comprehensive training and gaining practical experience in two 6-month placements. I manage a small caseload, working closely with individuals to promote enablement and recovery. Responsibilities include co-producing support plans to assist clients in achieving their aims and ambitions through creating SMART goals, liaising with and supporting client engagement with specialist agencies as appropriate, such as substance treatment services, community mental health teams and GP, etc., contributing to the meeting of the targets in the areas of Education, Training and Employment /move on of the service users, maximising service user involvement /and other Key Performance Indicators, engaging in any crisis intervention work and liaising with and supporting emergency services.
